Transition Plan Documents for New Federal Home and Community Based Services Rules

*10/24/14 UPDATE TO THIS SECTION: A notice relating to mandatory webinar sessions for HCBS Residential and Day Program service providers is now available. The notice contains a list of required participants, as well as dates, times, and additional information for the webinars, which will train Residential and Day Program service providers on how they will complete and submit their Provider Self-Assessments and/or Transition Plans.

*10/28/14 UPDATE TO THIS SECTION: Non-Residential and Residential Provider Self-Assessment Tools are now available for providers to use to determine compliance with the HCBS Settings Rule. A copy of the Provider presentation regarding how providers can complete and submit the Non-Residential and Residential Provider Self-Assessments and the validation process for these self-assessments is also available.

*11/3/14 UPDATE TO THIS SECTION: The Non-Residential and Residential Provider Self-Assessment Tools have been revised and the finalized tools are available below. Providers will also receive word versions of these documents from either a Managed Care Organization or the Department of Intellectual and Developmental Disabilities, as applicable.

*11/7/14 UPDATE TO THIS SECTION: The final version of the Transition Plan, which has been submitted to CMS for approval, and a revised version of the Residential Provider Self-Assessment Tool, have been posted below. Additionally, we are adding the Department of Labor-Home and Community-Based Services Provider Training Invitation for the Provider Trainings that were conducted in July 2014 regarding the new Department of Labor and CMS HCBS Settings and Person-Centered Planning Rules.

*11/13/14 UPDATE TO THIS SECTION: The final version of the Transition Plan was approved by CMS on November 7, 2014. The Approved Transition Plan is included below. Additionally, the Provider presentation on the self-assessment process has been modified based on provider feedback and is included below.

*11/17/14 UPDATE TO THIS SECTION: A recording of the webinar for HCBS Residential and Day Program service providers is included below.

*11/21/14 UPDATE TO THIS SECTION: Revised Individual Experience Assessment and Person-Centered Planning Assessment Tools are included below. Additionally, the Employment Data Sheet and training for providers on the Person-Centered Planning Self-Assessments and Validation Process are included below.

*12/5/14 UPDATE TO THIS SECTION: A revised Employment Data Sheet is included below. The Individual Experience Assessment Tool has been temporarily removed from the site, and a revised version of that tool will be posted the week of December 8th, 2014.

*12/19/14 UPDATE TO THIS SECTION: The Individual Experience Assessment Tool is being reviewed by the Department of Intellectual and Developmental Disabilities and TennCare’s contracted Managed Care Organizations. The revised tool will be posted by the end of January 2015.
