NOTICE: TDMHSAS Central Office to Temporarily Relocate Effective Dec. 17

The Central Office for the Department of Mental Health and Substance Abuse Services will be temporarily relocated to the Metro Center area of Nashville, effective Dec. 17. The mailing address will be: 601 Mainstream Drive, Nashville, TN 37243. All phone numbers, fax machines, and e-mail addresses will stay the same.

The TDMHSAS Office of Licensure, Review and Investigation is responsible for protecting Tennesseans who need mental health, developmental disability, alcohol and drug abuse, and personal support services by applying the department's licensure rules.

The Office of Licensure, Review and Investigation functions to protect the interest of tax-paying citizens against unlicensed service providers, unsafe environments, inadequate education and training of personnel, physical and mental abuse and any unscrupulous acts deemed detrimental to the treatment and general welfare of persons with mental health/developmental disabilities issues, alcohol or drug abuse problems. This also includes those in need of personal support services.

Operating out of three regional offices -one in each grand region of the state, regional staff:

  • Conduct unannounced inspections and follow-up inspections of licensed facilities
  • Develop the supporting documents needed to issue licenses
  • Conduct investigations of reports of abuse and deficiency in operation of a facility or service
  • Maintain current files and appropriate records for all facilities and services licensed by the department
  • Provide consultation and technical assistance as needed to licensed and proposed programs

Licensed Facilities/Agencies: Licensure Changes Coming Soon

Effective December 1, 2012, TDMHSAS will no longer license facilities providing only ID/DD services. The Department of Intellectual and Developmental Disabilities will assume licensure responsibility for these facilities. If your facility/agency serves both populations, MH and ID/DD, you will need to have licenses from both departments. You should receive notification from TDMHSAS Office of Licensure shortly. New Rules Filed for Opioid Treatment Programs (OTPs)
