GOOD CAUSE

Good Cause for a delay in obtaining immunizations or checkups will be granted for the reasons listed below:

 

        The child is ill and the health care provider recommends delaying immunization until the child is well.

 

        A health care provider does not respond to requests for information to document immunization or checkups.

 

        A caretaker has made a good faith effort to schedule checkups or immunizations within one month, but has not been able to do so because of circumstances beyond the caretakerís control.

 

        There are other extenuating circumstances that could delay immunization or checkups.
